90 Minute Comprehensive Case Review Consultation
The 90 minute consultation is an excellent option for patients/families who are needing guidance and support but may not need or be able to afford ongoing nurse advocate services

Individual Nurse Home Visits (Multi-visit packages available)
Home visits by a nurse to provide nursing care as determined by the care plan. Care may include, physical/mental health assessment, medication education, administration and med organizer prep, collaboration with other members of patient's care team and other services based on individual needs.

Injection Visits
Nurse picks up medications from pharmacy per patient's prescribing provider's order and administers in patient's home or other location as needed. Medications administered include, diabetes/weight loss medications, LAIs (Long Acting Injectable) psychiatric medications, hormones, and addiction recovery medications

Full Case Nurse Advocacy
You will be connected with one of our experienced nurse advocates who will provide comprehensive support and guidance every step of the way from hospitalization to home care. Our nurse advocates will work directly with hospital/program staff, attend medical appointments and provide/coordinate any additional needed resources.
